Surgery center or hospital — what actually differs?
The right setting depends on medical risk, surgical complexity, facility quality, and the plan for handling an unexpected problem. Here is how the two settings compare in the published evidence, and the questions worth asking your surgeon.
Two places to have joint replacement
Ambulatory surgery center or hospital?
This decision is about the resources and operating environment around your surgery—not how many hours you remain at the facility. Both settings can provide excellent joint-replacement care when the patient, procedure, team, and facility are well matched.
A focused center built around scheduled surgery
What it is: A licensed surgical facility outside the main hospital that specializes in planned procedures for appropriately selected patients.
- Specialized workflow: teams repeatedly perform a narrower group of elective procedures.
- Efficiency: fewer emergency cases compete for rooms, staff, and schedules.
- Lower facility cost: studies of knee replacement find lower episode costs at ASCs, although your personal cost depends on insurance and network status.
- Smaller environment: parking, check-in, and movement through the facility may be simpler and more predictable.
- Comparable safety in selected patients: available studies report similarly low complication, readmission, and reoperation rates compared with hospital outpatient departments.
The important limitation: ASCs have fewer onsite resources for an unexpected major medical problem. Their safety depends on patient selection, protocols, emergency readiness, and a clear hospital-transfer agreement.
Broader resources immediately available
What it is: Surgery performed within a hospital operating department with access to a wider range of medical services and specialists.
- Higher-acuity support: intensive care, advanced imaging, blood bank, respiratory support, and multiple specialists are onsite.
- Complex-case resources: may be better suited to revision surgery, major deformity, anticipated blood loss, or unusual implant needs.
- Medical backup: often the more appropriate setting for significant heart, lung, kidney, bleeding, anesthesia, or other perioperative risks.
- Immediate escalation: an unexpected complication can be managed within the same institution.
The possible tradeoffs: A larger, less specialized environment may involve more operational complexity, schedule disruption, and higher facility charges. Hospital quality and joint-replacement experience still vary.
What the evidence supports
ASCs can offer comparable outcomes at lower cost—for the right patients.
A 2023 comparison found similarly low 90-day complications, readmissions, emergency visits, reoperations, and revisions after knee replacement at ASCs and hospital outpatient departments. A 2024 analysis of 47,261 knee replacements found lower 90-day costs for ASC patients, although observational comparisons are affected by selection because ASC patients are generally healthier. The takeaway is not that an ASC is always safer or better; it is that a high-quality ASC can be an efficient, lower-cost option without sacrificing outcomes when selection and backup systems are sound.
The safer question
Does this facility fit my risk—and can it rescue me?
An ASC may fit when your health is stable, the operation is routine, support systems are ready, and the facility has a tested emergency and transfer plan.
A hospital may fit when medical conditions are less stable, surgery is complex, or there is a realistic need for advanced resources immediately onsite.
